Citizens’ Initiative – Adverse Events Following Immunization – AEFI REPORTING SYSTEM

This form is to register AEFI for COVID-19 “Vaccine(s)” only.

Have you or your near and dear ones, experienced some health problems after taking COVID-19 injection (“vaccine”)? If so, you may please reach out to us, so that we can better understand the situation, and attempt to help you. Moreover, this information is being collected all across the country. Your participation is extremely important! It will help us ascertain the occurrence of such adverse events in our nation. Thus it will help all of us to better understand the risks.

We feel that the affected party (patient) and their family are most suitable to identify an adverse event; because they know their body in subtle ways that a doctor can never know. So, whether a doctor validates a claim or not, a victim (of mild or severe reaction) should be given an opportunity to report their observations and their adverse event(s) without any complications or hindrance.

A real-time count of reports received and the analyses will be made live over a period of time (without disclosing identities or contact details).
